Though bellwether is a metaphor, Im not going to post this as one; the original term has been so lost from our daily experience that the word seems a linguistic oddball (a word worth its own post). But what, anyhow, is a bellwether? Literally, its the leader of a flock of sheep, the one with the bell. That dates to at least the 15th Century, but its not very kind to my native state. Neither is the definition of wether: a castrated male sheep. Ouch. So lets get figurative here. The OED records the earliest metaphorical use also in the 15th Century, simply as a leader. In those uses, the bellwether was a person, not an event. I cannot recall, in US usage, that nuance. Today we mostly use the term in relation to elections, sometimes stocks, though an entry at The Grammarist provides a few other fine examples from American English. However one employs the term, it generally means an indicator or predictor of something likely to happen more broadly, later. Watch your spelling on this one. I have long misspelled it bellweather. The main time conjunctions are: when, as soon as, before, after, by the time, by. They are placed either at the beginning or the end of a sentence. When placed at the beginning of the sentence, the speaker is generally stressing the importance of the time indicated. For example: As soon as you arrive, give me a call. Most often time clauses are placed at the end of a sentence and indicate the time that the action of the main clause takes place. For example: I had difficulties with English grammar when I was a child. Place Clauses Place clauses define the location of the object of the main clause. Place conjunctions include where and in which. They are generally placed following the main clause in order to define the location of the object of the main clause. For example: I will never forget Seattle where I spent so many wonderful summers. Reason Clauses Reason clauses define the reason behind a statement or action given in the main clause. Reason conjunctions include because, as, due to, and the phrase that the reason why. They can be placed either before or after the main clause. Education Madison attended the College of New Jersey (which would become Princeton University in 1896) from 1769–1771. He was an excellent student and studied a range of subjects, including oratory, logic, Latin, geography, and philosophy. Perhaps more importantly, he made close friendships at New Jersey, included the American poet Philip Freneau, writer Hugh Henry Brackenridge, lawyer and politician Gunning Bedford Jr., and William Bradford, who would become the second attorney general under George Washington. But Madison grew ill in college, and stayed in Princeton after he graduated until April 1772, when he returned home. He was sickly most of his life, and modern scholars believe he likely suffered from epilepsy. Early Career Madison didnt have a vocation when he left school, but he soon became interested in politics, an interest perhaps stirred but at least fed by his continuing correspondence with William Bradford. The political situation in the country must have been exhilarating: his zeal for freedom from Britain was very strong. His first political appointment was as a delegate to the Virginia Convention (1776), and then he served in the Virginia House of Delegates three times (1776–1777, 1784–1786, 1799–1800). While in the Virginia house, he worked with George Mason to write Virginias constitution; he also met and established a lifelong friendship with Thomas Jefferson. Madison served on the Council of State in Virginia (1778–1779) and then became a member of the Continental Congress (1780–1783). Father of the Constitution Madison first called for a Constitutional Convention in 1786, and when it was convened in 1787 he wrote most of the U.S. Constitution, which outlined a strong federal government. Once the Convention ended, he, John Jay, and Alexander Hamilton together wrote the Federalist Papers, a collection of essays that were intended to sway public opinion to ratifying the new Constitution. Madison served as a U.S. Representative from 1789–1797. On September 15, 1794, Madison married  Dolley Payne Todd,  a widow and socialite who set the pattern for the behavior of White House first ladies for centuries to come. She was a well-liked hostess throughout Jeffersons and Madisons time in office, holding convivial parties with both sides of the Congress in attendance. She and Madison had no children, although John Payne Todd (1792–1852), Dolleys son from her first marriage, was raised by the couple; her son William had died in the 1793 yellow fever epidemic that killed her husband. In response to the Alien and Sedition Acts, in 1798 Madison drafted the Virginia Resolutions, a work that was hailed by anti-federalists. He was secretary of state under President Thomas Jefferson from 1801–1809. Embargo Act and the Presidency By 1807, Madison and Jefferson became alarmed at increasing reports on upheavals in Europe suggesting that Britain would soon go to war with Napoleons France. The two powers declared war and demanded that other nations needed to commit to a side. Since neither the Congress nor the administration were ready for all-out war, Jefferson called for an immediate embargo on all American shipping. That, said Madison, would protect American vessels from almost certain seizure, and deprive European nations of a needed trade that might force them to allow the U.S. to remain neutral. Passed on December 22, 1807, the Embargo Act would soon prove unpopular, an unpopularity that eventually led to U.S. involvement in the War of 1812. Dolley Madison famously stayed in the White House until she ensured that many national treasures were saved. The New England Federalists met at the Hartford Convention in late 1814 to discuss pulling out of the war, and there was even talk of secession at the convention. But, on December 24, 1814, the U.S. and Great Britain agreed to the Treaty of Ghent, which ended the fighting but resolved none of the pre-war issues. Retirement After his presidential term in office ended, Madison retired to his plantation in Virginia. However, he still stayed involved in political discourse. He represented his county at the Virginia Constitutional Convention (1829). He also spoke against nullification, the idea that states could rule federal laws unconstitutional. His Virginia Resolutions were often cited as a precedent for this but he believed in the strength of the union above all. He took a leadership role in the formation of the University of Virginia, especially after Thomas Jeffersons death in 1826. Madison was also a slave owner—Montpelier had 118 slaves at one point—who helped found the notorious American Colonization Society to help resettle freed blacks in what would become Liberia, Africa. Death Although Madison remained vigorous and active during his early retirement, beginning after his 80th birthday in 1829, he began to suffer from longer and longer spells of fever and rheumatism. Eventually he was confined to Montpelier, although he continued working when he could through the winter of 1835–1836. On June 27, 1836, he spent several hours writing a thank you note to George Tucker, who had dedicated his biography of Thomas Jefferson to him. He died the next day. Most psoriatic lesions also tend to be dry and glycerin, urea-based or other humectant moisturizers help relieve soreness and improve recovery especially when used in conjunction with keratolytics – agents that soften and shed the abnormal dead skin layers. Systemic treatments When more than 10% of BSA is involved it becomes impractical to rely on topical measures alone. It may take more than half an hour just to apply creams on large areas of affected skin. Hence, oral or injectable medications are required. Many of these drugs are able to reduce the skin’s cell multiplication or maturation rate eg methotrexate, hydroxyurea, retinoids. However many others suppress the disease process at a more fundamental level by suppressing the activity of abnormal white cells (T-cells) or by neutralizing the T-cells inflammatory chemicals (eg TNF a) eg cyclosporine and the newer biologic agents licensed for use in Malaysia. With the proper use of these systemic agents singly or in combination, it is often possible to control even the most severe cases of psoriasis. However, these systemic agents should only be used by experienced physicians as many of them can have significant side effects. For example, persons with liver disease or gastric ulcers should not be treated with methotrexate; women of child-bearing age should not use retinoids as it can cause fetal abnormalities; persons with high blood pressure or kidney problems cannot use cyclosporine. If a person with severe psoriasis has liver disease, is a woman of childbearing age, or has kidney problems what options then are available for him/her? There are 3 options – oral salazopyrine which only works in 50% of Asians, the injectable biologic agents, or phototherapy (see below). Biologic agents are the new kids on the block. They have a much better safety profile than the above mentioned oral drugs and can be used in persons with liver, kidney disease and women but unfortunately their high cost means relatively few can afford them. Even in the west, where such expensive treatments are reimbursable, the psoriasis must be shown to be non-responsive to conventional oral drugs, or organ toxicities with these drugs are present. Some of them are very helpful in controlling psoriasis as well as the disabling arthritis that affects 10-30% of psoriatics. Phototherapy (PUVA, UVB, nUVB) This is an excellent form of treatment with artificial UV light which is relatively free from serious side effects apart from a small risk of developing non-melanoma skin cancers with prolonged use. This risk is probably not significant in the setting of Asian skin types especially with the newer types of phototherapy equipment such as narrow band UVB (nUVB), and the excimer laser or excimer light. Another advantage of phototherapy is that it is a clean form of treatment, nonmessy and can give a long remission period of up to one year. The main drawback is that it does require visits to the phototherapy centre 3 times a week for 2-3 months. The good news is that most states in Malaysia now have phototherapy units to treat not only psoriasis, but also vitiligo and severe atopic eczema. Alternative therapies In this day and age where the emphasis is on evidence-based medicine (EBM), alternative therapies must prove its worth with hard core scientific evidence from well-conducted clinical studies. Many people with psoriasis who have suffered from the condition for years are, not surprisingly, hopeful for answers, breakthroughs, and even a â€Å"cure† perhaps. Unfortunately, there is no shortage of charlatans offering â€Å"cures† for psoriasis, often in network marketing schemes. One has to acknowledge the fact that psoriasis has a genetic basis, and unless gene therapy becomes a reality, there cannot be a cure, even with accepted treatments such as climatotherapy in the Dead Sea. Diets for psoriasis are as yet unproven, although an FDA-approved healthy diet with at least 5-9 servings of fresh preferably organic produce may be of help in improving one’s general health. Stress reduction strategies may help insofar as stress precipitates and aggravates psoriasis. Prayer has been shown useful in several medical conditions, and a strong faith life can only help, if only to give one the equanimity to deal with the turbulence of modern life. The future of psoriasis treatments There is considerable optimism in the search for better and safer treatments for psoriasis in the near future. We now have a much better understanding of the genetics and underlying immune derangements in psoriasis. It is only a matter of time before the tremendous amount of ongoing research bears better fruit that is sweeter and without the bitter aftertaste of side-effects. Indeed here are promising smaller molecules in the offing that are cheaper to synthesize. Watch out for this space. Psoriasis affects 2-3 percent of Malaysians. When you love someone you care not only care about them as a person, but also about their well being. When they are hurt, you feel hurt and when they are in pain you feel pain also. Their physical and emotional problems are not only theirs, but they are yours as well. To be in love means to care about that person so deeply that your life would not be complete without them. The fact that you cherish one person so much is a blessing to some, as well as a gift. The following fable tells of an orphan girl who had all the happiness in the world. â€Å"There is a wonderful fable about a young orphan girl who had no family and no one to love her. One day, feeling exceptionally sad and lonely, she was walking through the meadow when she noticed a small butterfly caught unmercifully in a thornbush. The more the butterfly struggled to free itself, the deeper the thorns cut into the butterfly from its captivity. Instead of flying away, the little butterfly changed into a beautiful fairy. He claimed she loved me and was extremely happy, but in actuality I believe she was in love with the idea of what I could do for her to make her happy. She was not in love with me because love does not mean being selfish. When someone loves another person, they are not always concerned with what they can get out of the relationship; they are concerned with what they can put into it. Being happy with a person does not always mean that you are in love. I think you find this most in young relationships. I know in my case I thought that I was so in love and that I could never find that certain someone to make me feel the way that person made me feel.
